Tolleson Public Library is proud to provide passport services to the community. We only process new applications, long-time (more than 15 years) expired applications and youth applications. We do not process passport renewals. For renewal information, please visit the U.S Department of State website at https://travel.state.gov. It is required that you please bring all older passport(s) to your appointment.

We highly encourage appointments for our passport services. Walk-ins are accepted when space allows, but are never guaranteed. Unfortunately, we cannot accommodate walk-ins that require hand-carry delivery for their passport at this time. Hand carry passports must have an appointment. Please read all of our Passport Rules and Passport Requirements before booking an appointment. Rules not followed or requirements not met may result in a cancellation of your appointment.

Who Can Get a Passport?

You can get a new passport if you respond "Yes" to at least one of the following statements:

  • I am applying for my first U.S. passport book or card.*
  • I am applying with my child who is under age 16.
  • My most recent U.S. passport was issued when I was under age 16. (It is required that you please bring all older passport(s) to your appointment.)
  • My most recent U.S. passport was lost, stolen, or damaged.
  • My most recent U.S. passport was issued more than 15 years ago. (It is required that you please bring all older passport(s) to your appointment.)

If you could not respond "Yes" to at least one statement, go to the U.S. Department of State's Renew Online page or  Renew by Mail page.

*You may not have to apply in person if you want to get your first passport card and already have a passport book, or if you want to get your first passport book and already have a passport card

  • Thursday: 10:00 am – 4:00 pm 
  • Friday: 10:00 am – 4:00 pm
  • Saturday: 10:00 am – 4:00 pm
  • Sunday: closed

Passport Rulespassport services tile

  1. Check in at the library desk for your appointment when you arrive.
  2. Each appointment is a 20-minute time slot. Please arrive on time and be prepared to complete your passport service within the allotted time.
  3. You may include up to one additional family member or associate in your appointment.
  4. Appointments can be scheduled online through our official appointment system. Please ensure you select the correct date and time for your visit.
  5. At the time of your appointment, please bring the following:
    • A completed passport application form. You cannot complete this form during the appointment.
    • Proof of citizenship (e.g., birth certificate, previous passport)
    • A government-issued photo ID (e.g., driver’s license) 
    • Passport photos that meet federal requirements OR indicate in your appointment request that you would like to take passport photos. See our Taking Passport Photos information.
    • Any additional documents as specified in your application instructions.
  6. Walk-in services are available as space allows. We will try to accommodate walk-ins whenever possible. Please ensure you have completed all forms and have documentation and payment ready. Please understand there may be a wait and/or cannot guarantee service depending on space availability.
  7. Passport appointments are in high demand. If you are unable to attend your scheduled appointment, as a kindness to others, please cancel or reschedule at least 24 hours in advance so that your appointment may go to someone else. To cancel passport appointment online go here. 
  8. Arrive on time and with all necessary documents. If you are missing documents, you may need to reschedule your appointment.

Passport Requirements

  1. Complete all necessary forms before your appointment time.
  2. Forms must be printed single sided and should be completed in black ink. DO NOT SIGN YOUR APPLICATION FORM.You will sign it in front of the passport agent.
  3. Provide proof of citizenship (original and a copy).
    • Certified birth certificate, passport, or naturalization certificate.
    • For minors 15 and under, child and both parents must be present.
    • For minors 16-17, child and one parent must be present.
    • Any original documents you submit will be returned to you via mail separate from your issued passport.
  4. Provide proof of identity (original and a copy).
    • Valid government issued picture ID.
    • For minors, parents must present valid ID.
  5. A Passport photo.
    1. You can bring a photo that meets federal requirements
    2. You can take your photo during your appointment and you can mark this option when you schedule your appointment. Passport photo service is available on-site for $10.00 per photo and is payable to the library by credit card, debit card, tap, cash, or check. There are no refunds on photo services. Retakes are free if required by the US Dept of State.
  6. Pay the associated fees.
    • Pay the U.S. Dept. of State by check or money order.
      • All payments to the US Department of State must be with a check or money order. No exceptions. These fees include the cost of the passport, passport card, any expedited fees, and/or 1-2 day delivery.
      • Check the Passport Fees on the federal website to calculate how much you need to pay.
      • NEVER include the execution fee or any photo fees in the total amount. The execution fee and any photo fees must be separate and are paid directly to the City of Tolleson.
    •    Pay the Tolleson Public Library.

      • Payments for the execution fee and any photo fees can be made by credit card, debit card, tap, cash, or check. Remember, you MUST pay the cost of the passport, passport card, any exepedited fees, or 1-2 day delivery by check or money order to the US Dept of State.
      • The Execution Fee is $35 per application.
      • If you included photo service in your appointment, it is $10 per photo.
